On June 1, 2009, the U.S. government implemented the full requirements of the land and sea phase of Western Hemisphere Travel Initiative (WHTI). The rules require most U.S. citizens entering the United States at sea or land ports of entry--including travel to and from Canada, Mexico, Bermuda, and the Caribbean region which includes 17 nations-- to have a passport, passport card, or other travel document approved by the Department of Homeland Security.
The Clerk of the Court is only an acceptance agent, responsible for receiving and forwarding your application to the Department of State, Passport Services. The U.S. Department of State establishes the guidelines for acceptance and how the passports are processed. Instructions for Application with the Clerk of the Circuit
NOTE: All applicants must appear in person for a passport, including minors.
If applying ONLY for a U.S. Passport Book:
For a First Time Passport Book Application you will need:
1. Completed application form (DS-11) completed in black ink
The DS-11 form can be obtained from the Clerk of the Circuit Court's office or by accessing the following website: http://travel.state.gov/passport/forms/ds11/ds11_842.html.
2. Passport Fees
If you are 16 years of age or older:
o $110.00 in the form of personal check, money order, or cashiers check (NO CASH OR CREDIT CARDS ACCEPTED) payable to "US Department of State".
o $25.00 in the form of cash, credit card, check, money order, or cashier check made payable to the "Clerk of the Circuit Court".
If you are 15 years of age or younger:
o $80.00 in the form of personal check, money order, or cashiers check (NO CASH OR CREDIT CARDS ACCEPTED) payable to "US Department of State."
o $25.00 in the form of cash, credit card, check, money order, or cashier check made payable to the "Clerk of the Circuit Court."
3. Proof of US Citizenship
o Certified Copy of your United States birth certificate with a raised seal (please note - hospital birth certificates are not acceptable).
§ Long form birth certificates are required for minors and must list both parents' names.
§ Original foreign birth certificate translated into English.
o Original United States Naturalization certificate.
4. Two Passport Photos
o Passport photo services are offered by the Clerk of the Circuit Court for the customer's convenience. Photos are standard passport size ( 2” x 2”), full color, and meet all the guidelines and requirements of the U.S. Department of State for acceptance of passports. No appointment is necessary; cost per set of two, including tax, is $10.70. Passport photos are also available at most printing supply stores or camera shops. Be sure to ask if they are passport quality photographs.
5. Form of Personal Identification
The following documents are generally acceptable if they readily identify the customer:
o Previous U.S. Passport
o Certificate of Naturalization
o Certificate of Citizenship
o Government employee identification card (Federal, State, municipal)
o Driver's License (not temporary nor learner's permit)
o Aircraft pilot's license issued by the Department of Transportation, FAA
o Official military identification card
A photocopy of the valid form of identification (ID), showing the front and back of the ID and provided on plain white 8 1/2 x 11" paper must be sent in with the passport application(s). Upon request, the Clerk's office can provide the customer a copy of the ID for a $1.00 service fee.
Note: If you apply at an acceptance facility and submit out-of-state primary identification, you must also present an additional ID document. For example, if you apply for a passport in FL with a NY state driver's license, you must present additional identification, which may consist of a Social Security card, credit card, or employee ID.

Passport Renewal Requirements and Fees
NOTE: The Clerk's Office does not process renewal by mail applications.
You qualify for renewal by mail if:
1. You can submit your most recent passport book and/or card.
2. Use the same name as in the previous passport book and/or card, or be able to present a certified marriage certificate or court order changing the name.
3. Your previous passport book and/or card was issued on or after your 16th birthday and was issued within the past 15 years.
Fees:
If ONLY renewing a Passport Book:
o $110.00 check or money order payable to "US Dept. of State".
If ONLY renewing a Passport Card:
o $30.00 check or money order payable to “US Dept. of State”.
If renewing a Passport Book AND Passport Card:
o $140.00 check or money order payable to “US Dept. of State”.
Time Frames and Expedited Service
Customers will normally receive their passports within 8 weeks from receipt of completed applications by the Passport Agency. For an additional fee, those wishing to receive their passport sooner may request expedited service. Expedited service guarantees processing within 7-10 business days from receipt of complete applications by the Passport Agency.
NOTE: The processing time for a passport can vary based upon the volume of applications received at the Passport Agency. It is not uncommon, during high volume months, for processing to take up to 10 weeks for a completed application and up to 3 weeks for expedited service. · $184.96 in the form of personal check, money order, or cashiers check (NO CASH OR CREDIT CARDS ACCEPTED) payable to "US Department of State". This fee includes $110.00 application fee, $60.00 Expedite Service Fee, $14.96 overnight return delivery.
AND
· $43.30 in the form of cash, credit card, check, money order, or cashier check made payable to the "Clerk of the Circuit Court". This fee includes $25.00 execution fee and $18.30 overnight delivery to the Passport Agency.
If you are 15 years of age or younger:
· $154.96 in the form of personal check, money order, or cashiers check (NO CASH OR CREDIT CARDS ACCEPTED) payable to "US Department of State". This fee includes $80.00 application fee, $60.00 Expedite Service Fee, $14.96 overnight return delivery.
AND
· $43.30 in the form of cash, credit card, check, money order, or cashier check made payable to the "Clerk of the Circuit Court". This includes $25.00 execution fee and $18.30 overnight delivery to the Passport Agency.
